If you are a NEW patient...
​1) You have seen Dr. Karlstad or another hand surgeon at Twin Cities Orthopedics within the past THREE (3) YEARS

OR

2) You have recently had surgery with Dr. Karlstad
1)  You have not been seen at Twin Cities Orthopedics in the past 3 years

or

2) You have not seen Dr. Karlstad in the past 3 years
